Default A4 rpms fluctuate after temperature reaches operating temp.

A4 has 110,000 miles and when its cold its fine? When its warm forget it. It seems like it shifts funny to? It seems like im hitting on the gas and letting go. When i use paddle shift its fine. Im not sure what trans i have but its auto/stick. Someone said i need a flush? Any body have this problem? Any outcome? Thanks!
